HomeFront OT Assessments HomeFront was developed by DVA to reduce the risk of falls and similar hazards by assisting in the provision of minor home modifications and appliances for all Gold and White Card holders who continue to live in their own homes. A free assessment is available once every 12 months. DVA contributes up to $207 per year, for the cost of those modifications and appliances recommended by the HomeFront assessor. That person will usually be a qualified Occupational Therapist. Gold and White Card holders in Doncaster, Templestowe, Donvale, Bulleen, Balwyn, Nunawading, Blackburn, Greythorn, Ringwood, Surrey Hills, Mitcham, Wonga Park, Warrandyte, Tunstall Sqaure, are invited to call 0409 025 233 to arrange a direct assessment. For all other area's call 1800 80 1945 to arrange an assessment. Rehabilitation Appliances Program (RAP) RAP is an Australian Government program, administered by the Department of Veterans' Affairs, which provides aids and appliances to eligible members of the veteran community to help them maintain their independence as they grow older. RAP appliances are available to gold and white card holders with a clinically assessed need. Aids and appliances are provided based on a referral from a local medical officer or health professional (e.g. an occupational therapist). RAP product groups are: 1. Continence 2. Mobility function and support 3. Oxygen 4. Diabetes 5. Personal response systems 6.Continuous positive airway pressure. 7. Medical Grade Footwear
