Achieve Disability Supports Pty Ltd provides, person-centred caring support services to to help people who need assistance. We work with NDIS, Aged Care, Veterans Affairs, Workcover and Children's Services. Based in Deception Bay, we have Support Workers that service the Moreton Bay area mainly, however we can also help people in other areas such as Brisbane, Ipswich, the Gold Coast, Sunshine Coast, Toowoomba and surrounding South-East Queensland areas. We have support workers that work with, understand, and respect the INDIGENOUS, LGBTIQ, CALD (Other cultures), and other minority groups. Our services include personal care, household tasks, daily living skills, support coordination, and community access. We can take you places you enjoy like the shops, the movies, shopping, bowling, fishing or to go to appointments or see your doctor. We support participation in school, work, and everyday activities. We also offer several types of accommodation: Short-term (STA), medium-term (MTA), Respite and leaving hospital accommodation, and longer term if NDIS or Aged Care recipients. We also assist with NDIS applications, plan reviews and connecting you with allied health providers and support services. Whether you need help with daily activities, greater independence, or community access, we are here to help you make the most of your NDIS plan. We have 2 houses that are in Deception Bay, where we host respite and other living arrangements. Office hours: 8 am–7 pm Monday to Friday, 9am - 4pm Saturday to Sunday. 24-hour emergency supports available if necessary. Please call us on 0431 227 255 or email [email protected]
